You or a loved one may benefit from home health care if:
• your doctor ordered a wound vac for your wound
• you need monthly foley cath/suprapubic catheter change
• you have a new feeding tube and need teaching
• you have a PICC/Central line and needs weekly dressing change while on antibiotic(s)
• you had recent changes to your medications, trouble remembering if you took your meds or not
• you have difficulty leaving your home
• you fallen in the past 60 days
• you have trouble maintaining your blood sugars (diabetic patients only)
• you had difficulty standing or walking in the past 60 days

• you have open wounds on your body your ankles have been swollen more than usual

If you have a diagnosis that requires skilled care, instruction, and/or management, home health care may be for you. In order to qualify, you must meet the homebound status criteria and have a current referral from your physician. Check with your insurance provider to see what home health benefits your plan offers.
You may continue to receive home health services as long as it is deemed necessary by your clinical team and physician. As long as you continue to meet the homebound status criteria and have a medical necessity for care, you will continue to qualify. be sure to check with your insurance to see if they have a cap on services under your plan.
